Book orientation simply has to do with being able to recognize the top, bottom, front, back, et cetera of a book. If you hand a child a book and they insist on reading it upside down, call for your local priest immediately, your child is possessed. If you hand a child a book and they start reading it from the back, they may just be being ornery. Seriously, if a child really has trouble with this concept, read them a book...Show them the cover, have them say cover, say it is also the front, have them say front...and so forth and so on...Next work with the child to create their own book inspired by the book you just read.
